I have several patients with an early stages of dementia and I do a lot of teaching (home health nursing) I have been told to be extra careful when documenting teaching given to a patient with an early stage of dementia who are often forgetful.Thanks

In the LTC I worked at I would document exactly what I taught and the patients reaction, memory of the teaching. I would also state what the patient had problems remembering and how long the teaching time was. Hope that helps.

Whenever possible try to have a family member or responsible party present for the teaching, and document who was there. Also, if possible, have printed handouts for everyone to refer to.
I can almost guarantee that there will be little remembered by either of them.......
