Another COT question

Quick question

Would a COT be required in this instance:

5 day RUG is RMA10

14 day RUG is RMA2D

Same RUG Category. Would a COT be required if the RUGs were different (i.e RMA vs. RUC)?

Featured Replies

A COT is only necessary when the RUG category is different. Whether or not you complete a COT, you must still assess for another COT after 7 more days. RAI Manual pg 2-41

So, a COT is not needed if the RUG merely reflects increased therapy minutes as long as the Resident remains in the Rehab category?

I will answer your question by directing you to the "August 23, 2011 National Provider Call Follow-Up and Clarifications" on the CMS RUG Training page at https://www.cms.gov/SNFPPS/03_RUGIVEdu12.asp

There you will find a host of answers to some of the trickier, and not too tricky, questions involving the setting of ARDs, EOT-R, COT, LOA, and more. Anyone who has not read that document is probably in for a number of surprises regarding MDS. Please read it if you have not.

This sounds like 2 questions so...

1) No COT required..same RUG.

2) Yes COT required, an increase in RUG, likewise, a decreased RUG would also require a COT assessment. Of note, the ADL score has no berring on the COT decision.
