Analysis of a nursing problem using EBP & Theory

I have a question regarding an analysis of a nursing problem using EBP and theory. I have to fit it to my scope of advance practice nursing program. I am going to use dementia and patient-centered care and the ongoing lack of patient care in my facility. I'm not sure how to start the beginning of the paper. Do I begin with information about the facility, the patients, staff, stakeholders and then give a short explanation about what dementia is? Then add the model i've chosen (Iowa model).
