Accidental migration of PICC line (1-1.5cm)...

Published

A new PICC line, changed the site that the line had migrated about 1-1.5 cm out from the exit site. Still prefect blood return.

I was all thumbs and had a issue with the Cath secure.

Advice? :/

(So about 2.5cm from proximal end of the wing)

What's your hospital policy? Ours allows 3 cm migration in either direction before requiring re imaging. The concern with picc lined migrating isn't about blood return but that it is no longer in the right spot (sup vena cava)

Definitely check out hospital policy because that's what'll get ya.

That said, If you needed the line is should be perfectly fine to use. Maybe have some caution with things like vasopressors.

As long as you resecure it, a picc line thats out even several cm with a good blood return is still a darn good PIV in my opinion.

Yea, what Georgia said.

Ideally the tip will terminate in the lower 1/3 of the SVC or CAJ. I would look at the last CXR to determine where the tip was. If it was deep in the SVC/CAJ then 1.5cm is probably inconsequential.

But... better to be safe.
