Just at a quick glance, there is some issue/mistake with per diem.

I thought it was low compare to all the other contract she sent me, but is this normal to have two different contract for the same assignment, just because the agency said she the contract would be for 36 but sometimes 48 hours a week clause in there? And mind you I got this package 48hours after interviewing with the facility since the recruiter never even send me anything before hand when I asked her to.

I have no idea why agencies do the things they do. It seems simpler just to define hourly pay and overtime pay and then only one set of numbers is needed. But if you want to know why they do what they do, you will have to ask them.
