I have a friend who is starting school in January at UTA. She just had a baby and her Hep B series won't be complete. The secretary told her she could fill out a form, fax it and the state would send a waiver. Anyone know where to find this info?

I know nothing regarding the form you have mentioned.
However, your friend can possibly have the hep-b requirement waived by having a titer drawn. Even if your friend has not finished the series of shots, (s)he might display some immunity, and a titer is the only way to find this out.
I had not completed the hep-b series prior to starting school, so I had a titer drawn. It indicated that I had developed immunity, so the vaccination requirement was waived.

I think that your friend was misinformed.
The only way I know of to get a waiver is to have a titer drawn that shows that she has an immunity. It is possible that some schools will allow you to decline the Hep B series (bad idea in my humble opinion) but that would have to come from the school.

I know that there is a spot on the UTA immunization form for you to put down that you are requesting a HEP B waiver. Also the rules for UTA (if I remember reading everything correctly) just state that you have to have your first injection by the time you start clinicals and have the series finished within 6 months.
