At your facilities--How do the recommendations of Physical,Occupational or Speech Therapy become part of what the nursing staff follows when giving care?
Do the therapists give the recommendations to the Dr and the Dr then writes orders for nursing staff to follow or does your nursing staff follow the orders directly from the therapists?
Our PT /OT have direct communication with the satff on unit. Physician certainly writes order for PT/OT therapy, but as far as unit recommendations, they are just that. The doc does not order unit ambulation, ot the number of assist needed for an interventions. It is in out P & P to follow the recommendations of PT/OT after therapy completion or concurrently.
The PT/OT here use a sticker in the chart, formatted wording that indicates transfer, ambulation, devices, recommendations, referral to nursing rehab etc. OT has a similar sticker, areas for commode, ADL recommendations Nursing Rehab referral..
The nurse must sign the sticker under the therapist, by signing you are stating that you are transferring the information to the CNA information sheets.
hope this helps.
