I was taught that voices inside the head were thoughts, external voices were auditory hallucinations.

I am no psych expert, but I certainly hope you're right....I "talk to myself" all the time (inside my head) :chuckle . If memory serves me correctly, when the person "hears voices" they actually "hear" someone talking to them..not just voices inside their head. The voices usually are "different" than their own thoughts, and even a different gender sometimes.

Two different sx pictures: 1) pt complains that he hears people talking about him but they always deny it and sometimes there is not even anyone there when he looks. He is inclined to think someone is playing malicious tricks on him.

2) pt complains she hears voices, which she does not identify as "her" thoughts, that she says are infact not her, but which she identifies clearly as being inside her head. Voices often are critical of her and her behavior and may argue with each other.

1) is an example of psychotic process. 2) is most likely dissociative process
