Collaborative care for nursing a diagnosis

Hello I am having a hard time coming up with some collaborative care for some of the nursing diagnosis for patients in the psychiatric unit.

1. I have sleep pattern disturbance r/t bipolar 1, fatigue.

2. Impaired verbal communication r/t slow thought process.

3. Altered thought process r/t biochemical imbalances for sensory distortion ( hallucination).

4. Impaired social interaction r/t sleepiness.

Common Plans of Care for my mental health patients include:

Altered Mental Status r/t Psychosis.

Impaired Safety and Fall Risk r/t AODA Withdrawal.

Ineffective Coping r/t Depression and Anxiety.

Impaired Nutrition r/t Catatonia.

For your first example, fatigue is a symptom of sleep pattern disturbance, so using an example such as mania would be a better fit.

For your first example, fatigue is a symptom of sleep pattern disturbance, so using an example such as mania would be a better fit.

That, on another forum, sounds similar to my reply:

Sleep disturbances are typical with patients diagnosed with bipolar when they're in their manic phase and fatigue is rarely their complaint because they're so juiced.

I believe ours is a case of someone working hard to avoid work. Getting the answer, and not learning, is the goal here.

I was just asking for ideas but I guess this place is just for critizing people. Thanks for your help. I already finish my homework and I did learn. There's nurses like you that once they become nurses they forget they were student too and put student down for asking for help.
