What are your most sad loss of patient stories?

One of my most sad experiences was when I was working in a neonatal intensive care unit and heard the loud moans of a grieving Mom who was watching her dear little one slowly pass away in his incubator in our unit! Parents who are losing their child in a NICU are always sad and often express their grief quietly or softly, but this dear Mom was so totally overwhelmed that she just lost it! The neonatologist had ever so kindly explained to this Mom that we had done every single thing we could do to help her child and that there was unfortunately nothing else we could do. When we heard this sad parent's gut wrenching loud cries, every single one of us staff members had silent tears! We were all visably shaken as we watched and listened and shared this sad grieving experience. Hard stuff!
