I'm newly joined NHS nurse,and I'm looking for a experienced nurse to explain some of the issues that I encounter in my daily working day.In an ideal word I could ask one of my more experienced colleagues at work,however time not always allows for guidance to be given .Therefore I would grateful if one of you could spare a few..,to explain what would you do if...

What clinical area are you working in?

What specifically are you asking about?

Clinical questions, staffing ratios, role of band 5, benefits, training . . . . . ?

Hi skylark,

I'm working on cardiology and respiratory ward.

At the minute I'm a bit stuck in sorting out discharge planning for elderly who can't cope home alone,and what are the services available that can help them with this.And who is responsible for organising this,is the nurse in charge or the discharge coordinators.

That depends on the set-up at your hospital. Discharge team will generally facilitate discharge by locating a bed or facility, or arranging home care. But the nurse for that patient is usually responsible for the actual transfer, arranging TTOs, transport, faxing notes, etc.

But I would advise meeting with the discharge team yourself and getting their take on it, it might be completely different : )
