Teaching/learning care plan?

A requirement for our class is to have regular discussions/postings on this website called Springboard (I don't know if any of you are familiar with it), usually twice/week. We have been broken off based on our clinical groups. Earlier in the semester each of us had a drug that we were supposed to look up and we are using that same drug for this teaching/learning care plan (mine was digoxin) and nursing diagnosis for this teaching/learning care plan is deficient knowledge: digoxin. They never really went over teaching/learning care plans, so I'm a little lost, but here is what I have come up with so far:

GOALS

* Pt will be able to explain the purpose of digoxin

* Pt will be able to recognize s/s of digoxin toxicity and when to contact their doctor/healthcare provider

* Pt will be able to take their own pulse

CONTENT OUTLINE

* Teach how to take pulse (tell pt to contact their doctor/healthcare provider before taking medication if pulse is 100bpm)

* Review s/s of digoxin toxicity (first signs are usually abdominal pain, anorexia, N&V, visual disturbances (yellowish tint to vision or halos around lights), bradycardia or other arrhythmias; weakness, confusion, dizziness; advise pt to notify doctor/healthcare provider immediately if these or sx of CHF occur. Inform that these sx can be mistaken for the cold/flu)

* Drug safety (avoid taking w/ meals bc meals can decrease absorption; avoid concurrent use of OTCs & herbals without first consulting your doctor; avoid antacids/antidiarrheals w/in 2 hours of digoxin)

* Review fall prevention strategies (adequate lighting, keeping areas free of throw rugs, cords or wires) - digoxin has been associated with increased falls in the elderly, and the patient in this scenario is an 84-y.o. woman

TEACHING STRATEGIES

* lecture on s/s of digoxin toxicity, drug safety & fall prevention strategies (maybe give handouts?)

* demo/return demo with pulse

My drug book also said under teaching considerations: "weigh yourself every other day with the same clothing at the same time and record weight." why is this? Is this because digoxin toxicity can produce s/s of CHF (edema)?

Am I missing anything here? I either feel like I should add something else or I have waaaay too much, but I don't know. Any help would be appreciated! :bowingpur

what you have is pretty good with some problems which i addressed in purple, gave you some websites and what goes into a teaching plan which you already seem to know.

goals

* pt will be able to explain the purpose of digoxin - you have no interventions that include teaching the purpose of this drug in your content outline so you can't legitimately have this as a goal

* pt will be able to recognize s/s of digoxin toxicity and when to contact their doctor/healthcare provider

* pt will be able to take their own pulse

content outline

* teach how the this drug works on the heart to correct the heart failure

teach how to take pulse (tell pt to contact their doctor/healthcare provider before taking medication if pulse is 100bpm) - also teach

look for the reasons that digoxin is prescribed for patients. go to outside resources such as medline plus which is a consumer website and see what they have to say about digoxin for th public. digoxin is usually given to patients that have chf to help improve the pumping ability of their heart (cardiac output). it would be noticed by their edema improving, thus their weight going down due to fluid loss.

a written teaching plan goes something like this:

  1. overview: a synopsis about what is going to be taught in the course
  2. goal(s): the aim(s) or outcome(s) that you want your learner to achieve as a result of the lesson you plan
  3. objectives: the more specific information that the learner will come away from the course knowing that will achieve the goal(s) you have determined.
  4. content: a play-by-play of the specific content that is going to be taught and in the sequence it will happen. your content should address and cover all the objectives. this part of the written lesson plan is presented in an outline format.
  5. procedures and materials: how all the above will be achieved, i.e. lecture, demonstration, discussion, etc. materials that can be used and resources that can be needed for the lesson to be successful and essential to teaching your lesson plan are listed and may include demonstrations, audio-visuals, handouts, experiments, stories, game playing and any number of other creative items.
  6. evaluation: determining if you met the goals of the teaching plan. this can be done through a return demonstration, short post test, short question and return answer session with the client to verify they understand the information correctly or a task the participant needs to perform.

if you look at it, it has some of the elements of a care plan (goals, interventions, execution and evaluation). what is different is that you actually lay out how a list of how you are going to do the teaching, kind of like a nursing procedure is laid out step-by-step for you.

Hey there, so I'm doing a similar assignment, and you are definitely on the right track but.... for our learning outcomes (aka goals) we are required to have a time frame that the goal is to be achieved by, as part of the goal and they also must include a measurable or an observable activity, just like client outcomes for nursing diagnoses. Also, we have to address one of the three "learning domains" in each goal, and they are: cognitive, affective and psychomotor. For example, a goal/learning outcome for the 'cognitive' domain would be: By discharge, the Pt will state three of the s/s of digoxin toxicity.

And for the 'psychomotor' domain: At the next office visit, the client will demonstrate the ability to correctly assess his/her pulse.

Hotdharma, I totally forgot to include the "measurable" aspects of my goals! I was focused too much on getting the whole care plan together...thank you so much for reminding me of this!
