Here is the problem w/ my answers. Please help me if I'm doing this all wrong.....Thanks

A 2 year old, 10 kg patient has Fortaz 450 mg q 8 hours IVPB ordered. Dilute to a concentration of 40mg/ml. Infuse over 15 to 30 minutes.

How many mg/day is your patient receiving? 450mg x 3 = 1350mg/day

Calculate the actual daily dose (mg/kg/day). 1350mg/day divided by 10kg = 135 mg/kg/day

How much fluid should be added to the powder in a 500mg Fortaz vial to reconstitute to a concentration (undiluted volume) of 100mg/ml?

500 mg times 1 ml / 100 mg = 500/100 = 5 ml

What volume (ml) is needed for one 450 mg dose of Fortaz diluted to a concentration of 100 mg/ml?

450 mg times 1 ml / 100 mg = 450/100 = 4.5 ml

Calculate the total volume needed to dilute this medication to a final concentration of 40 mg/ml.

450mg times 1 ml / 40 mg = 450/40 = 11.25 ml

Calculate the IV pump rate to deliver the Fortaz (final concentration of 40 mg/ml) over 20 minutes.

11.25 ml / 20 minutes times 60 minutes / 1 hour = 675/20 = 33.75 or 34 ml/hr

I feel like I'm missing a lot of steps. Any help would be appreciated :heartbeat

That all seems correct to me, no steps missed at all. The only thing, when you got 11.25, I would just make it 11.3ml. That would make you get 33.9, which is still 34ml/hr. But you didn't miss any steps. :up:

Make sure you know what your school does for rounding rules. Our school would leave all answers to the hundredths since this is what you would see in reality on a Peds floor. IV pumps can be programmed to run in hundredths and medications can be drawn up to the hundredths spot using an oral syringe.

That's a good point. Even at my school, some different teachers want their OWN rounding rules to apply on their tests.
