Pediatric Health History...

We have to write up a health history for peds on a child and I have gathered all my information but I am having trouble figuring out the proper format to write it. I think my teacher wanted something that was more of a narrative format (like how the health assessment would like)...but whenever I try to find information on how to write a health history I just see the new "fill in the blank" papers that you get at the doctors...

Anyone know of websites that have examples of well written health histories??? Or know any suggestions themselves? Any help would be greatly appreciated!!!

your write up is very similar to the history and physical documentation that is done by a physician. you can get instructions on how to write up a physical exam and review of systems on this website:

at the top of the page are links to the different areas covered. click on the link "write ups" where you will find two examples of how to write up a history and physical.
