I just started nursing school to get my lpn & I really need help.. My clinical instructor had us assess are patients & come up with 5 nursing diagnosis based off of 1 of the patients medical diagnosis and we have to prioritize them & make a care plan out of the top priority. My pt had a diagnosis of MS so after doing my assessment I came up with ACTIVITY INTOLERANCE, IMPAIRED MEMORY, IMPAIRED PHYSICAL MOBILITY, SELF CARE DEFICIT,& RISK FOR FALLS.. please help

However, if there is no additional information about the patient, this is how i would prioritize it..

Activity intolerance which is mostly associated with impaired gas exchange such as coughing and difficult breathing

Self care deficit..aeb...

Imparied physical mobility

Impaired memory

Risk for fall

Specializes in PICU, Sedation/Radiology, PACU.

Just like when you're prioritizing care, prioritize your nursing diagnoses based on the ABC's. Then ask yourself which ones are causing the most immediate risk or compromise to the patient? The specific order depends on your assessment findings.

Patients with MS have a severe activity intolerance. Simple tasks are exhausting.
