I have to do a care plan with assessment, 2 nursing diagnoses, 3 interventions for each diagnosis, and a goal for each. my patient is a 6 day old male who was kept in the hospital for jaundice. bilirubin levels after being under the light were running around 11.5. Today they went up to 13. He was discharged with a bili blanket. so for my first diagnosis I used neonatal jaundice. I'm stuck on the 2nd one. I want to do something with breastfeeding. His mom was discharged 24 hrs after delivery and was bringing breast milk and thats what the baby was eating. I dont think it would be considered ineffective breastfeeding because it wasnt ineffective, the baby just wasnt given the opportunity to latch. So would you do a breastfeeding care plan d/t the mom wanting to breastfeed but not being able to since the baby was in the hospital or would you do a coping care plan? he really didnt have much going on. His vitals were stable. He was circumcised and that looked good. This is the womans 5th baby so thats another reason that i kind of want to go with coping. like a family process type thing.
