Long Term Care Plan Goal!

Hi everyone! I am a second year Nursing Student and I am writing my second care plan. I would really like some help/ input on a long term goal (such as in multiple shifts) for the following diagnosis.

Impaired Gas Exchange related to,

Pulmonary congestion, secondary to alveolar- capillary membrane changes, and ventilation perfusion imbalance

as evidenced by,

Dyspnea on exertion, shortness of breath, states it is, "hard to breath in", and some inability to move secretions.

We have to have 2 short term goals and 1 long term goal in the smart format (specific, measurable, attainable, reliable, and time measured).

For each goal, we then must explain 2 independent and 1 dependent intervention to meet the goal

I have completed my 2 short term goals which my instructor already said were good

For an example, One of my short term goals are ... By the end of the shift, patient will be able to breathe deeper and promote coughing by demonstrating use of incentive spirometer

and showing decrease labored breathing. My independent diagnosis are to raise HOB to semi-folwers and to promote use of incentive spirometry. My dependent intervention for this goal was to administer b-2 agonists as prescribed.

The other short term goal I have involves maintaining clear lung fields and remain free of signs of respiratory distress throughout the shift... with administering O2 as the dependent intervention, and encouraging suctioning and encouraging pursed-lip breathing as independent

I know this post is long, and I am not asking anyone to do work for me, however I would very much appreciate any input on creating a long term goal /any following interventions.

What sort of assessment to you and your patient hope to have after several shifts? Ability to ambulate, perform ADLs, take meals in dining room or attend activities ? Perhaps you and your patient want to return to or exceed baseline functioning (if that is attainable), but you will need to describe that.
