:heartbeati need help, i desperately need a careplan for my project. im an LVN student my instructor ask us watch the movie "the Notebook" and correlate to Alzheimers Disease and make a reaction paper for the movie that will say something about alzheimers disease . and she wants us to make a careplan short term and long term goal for the woman in the movie who has Alzheimers Disease. pls. i really dont know how to do this. i need at least guidelines for this project.

Hi, I always did very well on my careplans, and was given the following format to follow:

one r/t pt safety r/t disease

one r/t pt psychological/social needs also r/t disease

and One on disease process or other outstanding needs demonstrated by the pt and how it impacts that pt.

It made it easier for me to then look up nursing dx and r/t to the care plan when I knew what to look for, not just the whole nursing dx book.

So if you've got AD, not knowing the pt in the movie, you might have one r/t pt falls if there is some concern there (safety).

And one on social interaction, with the memory loss (psychological/social)

and depending on what that particular pt may need, maybe fluid deficit if they are having trouble swallowing or remembering to drink fluids. Again, it would depend on the individual.

to write a care plan you must follow the steps of the nursing process in the sequence that they occur. those steps and what goes on in each of them are:

  1. assessment (collect data from medical record, do a physical assessment of the patient, assess adl's, look up information about your patient's medical diseases/conditions to learn about the signs and symptoms and pathophysiology)
  2. determination of the patient's problem(s)/nursing diagnosis (make a list of the abnormal assessment data, match your abnormal assessment data to likely nursing diagnoses, decide on the nursing diagnoses to use)
  3. planning (write measurable goals/outcomes and nursing interventions)
  4. implementation (initiate the care plan)
  5. evaluation (determine if goals/outcomes have been met)

since this is a "made up" patient the only information you have to work with is what you saw in the movie and that the character in the movie had alzheimer's disease, is that right?

the first thing you need to do is what step #1 of the nursing process directs you to do--assess the patient. in this case you can kind of combine the assessment with step #2's making of a list of symptoms. what kind of abnormal behaviors did the lady in the movie have? you need to make a list of them. you also need to look up information about alzheimer's disease. if you do not have any books with that kind of information you can find information about alzheimer's disease in the weblinks listed on this thread of allnurses:

the reason for doing this is to learn more about alzheimer's as well as to double-check yourself to make sure you didn't miss any of the symptoms the character in the movie might have had. it is important to gather as many symptoms together as you can remember because the foundation of your care plan rests on the symptoms the patient displayed.

once you have this list of symptoms, then you can get down to the business of writing the actual care plan. first, you want to identify the patient's problems, or choose nursing diagnoses. the best way to do this is to have a nursing diagnosis reference of some kind. every nursing diagnosis has a list of signs and symptoms. your patient must have at least one or more of those symptoms before you can assign a nursing diagnosis to them. most current nursing care plan books have this information listed in them. you can also get some limited access to free nursing diagnosis information you can link into on post #109 of this thread on the allnurses nursing student assistance forum: https://allnurses.com/forums/f205/desperately-need-help-careplans-170689.html - desperately need help with careplans. if you are having trouble getting through this step #2 of the care plan process, post the list of this character's symptoms and i will help show you how to choose the nursing diagnoses for her.

once the nursing diagnoses have been chosen you can move on to step #3 which is to write nursing interventions and goals. these are also based upon the symptom list. nursing interventions can be found in your nursing textbook(s) or care plan books. goals are the results you expect to see when the nursing interventions have been performed. so, the goals and nursing interventions are very closely linked with each other. you can see how to formulate goal statements on this post: https://allnurses.com/forums/2509305-post157.html
