I've written three careplans in my life - none for pediatrics and none outside a hospital/long-term care setting, so I'm stooped! I have a toddler born a premie with no "medical" interventions besides 02 (prn) necessary, only developmental delays. The setting is a skilled-nursing day care center (I didn't even know they exist!). I'm having difficulty with NANDA diagnoses as I've only been able to find failure to thrive so far. The little one is behind on basic developmental milestones: doesn't have a vocabulary, doesn't repeat words to mimic, can't id parts of body, etc, etc, etc....basically I need to write something that addresses cognitive, motor and speech development. I'm not sure how to approach this...direction would be appreciated.

This is why a nursing diagnosis reference is necessary. I can give you some diagnostic labels, but you will still need to find a nursing diagnosis reference book so you can look these up to understand what they mean and how to use them:

  • Delayed Growth and Development
  • Impaired Social Interaction
  • Impaired Verbal Communication
  • Disturbed Thought Processes
  • Any of the Self-care Deficits
  • Risk for Disproportionate Growth

I did clinicals in a long-term pediatric care facility as a student. Many of these children also have physical problems as well, so many of the same diagnoses used in nurses homes can also be used (nutrition, mobility, incontinence).
