Careplan and Maslowe

Hi.

i normally am pretty good at doing careplans, but I am getting stuck this time! I am overthinking, maybe. I need a careplan with at least one dx from each level of Maslowe's Hierarchy.

4 yr old female no c/o pain

polynephritis has very positive attitide

suprabubic small for age

mother rarely vsits, maybe once per week, client asks for mom constantly presented at ED w/ 105 fever, distended abd, uti, for over a week

Picc line 4 yr old very intelligent

TPN d/c this week, but she has very little appetite

foley

I am using altered nutrition for level one

using at risk for infection for level two

growth and development delay for level five

I am stuck on levels 3 and 4, thought about altered parenting but am not sure how to back it up appropriately

Any help would be greatly appreciated, I always have problems with the psycho social levels.

Well, let's see.....You said you were thinking of using impaired parenting. You can back that up by stating that the kid wants her mother there, and the mother hardly ever comes to see her. She also seems to have a failure to thrive issue based on being small for age.

As far as level 3 goes you could look at risk for loneliness, risk for impaired attachment, too.

Level 4. Powerlessness might work since she wants to be with her mother badly and she can't control the fact that her mother is not there. Post-trauma syndrome from being in the hospital? Just a thought.

Thank you for your suggestions. I guess I am at the point where my brain feels as if it will explode! I am usually the one that others come to for help, and I think it might be the pediatric angle that is getting to me.

Sounds like some possible neglect issues with the abdomen distended, UTI, fever for over a week.
