bed sores

Today a residents family member called the state to my facility. It is her belief that the patient was not handled properly concerning a sacral bed ulcer. to begin with treatment was started on what appeared to be a stage two. standards of care such as T+P , diet supplements ( due to poor intake) , new w/c cushions where all established . however the ulcer progressed . until a wound specialist was called in . perhaps 2 weeks into this situation . the wound specialist debrided the ulcer and well lets say it was deeper than we had thought or was able to identify. of course there has been complications such as sepsis. its all really unfortunate. however , my question is . If I maintained standard of care . Am I still to be held responsible for this pt's pressure sore and if so what could possibly happen to my license.
