Hello everyone-- The practice that I have just joined has not used employment contracts ro collaborative agreements in the past, and I need to build one but I have no idea how. I need a template or some hard factors in which to include. I am a new grad and I want to make sure that all is well for me here and onward.

Your state BON may have specific requirements regarding physician-NP collaboration. Therefore, I suggest you check with someone at your state's BON to find out if there are certain elements that need to be included in the document. There are also specific requirements set forth by Medicaid and Medicare and they also need to be included in the document. My state nursing association has a guide for APN's to follow when writing collaborative agreements based on the requirements set forth by the Michigan Public Health Code regarding NP's and how these rules apply to our practice. I couldn't find a similar link for Connecticut but it looks like the APRN practice act in your state is found here: http://www.ctnurses.org/displaycommon.cfm?an=1&subarticlenbr=12. I would read through these documents and make sure your collaborative agreement document complies with the rules stated in the said link.

I would be surprised if your program didn't have any examples of collaborative

agreements...

I apologize for being so sketchy but the agreement will probably vary widely from state to state, by provider, by situation, etc...
