I am working on an assignment for school which asks for 4 essential elements of a written collaborative agreement btw physician and APN. I don't know if it is specific to PA or nationally. I have searched everywhere (NP association websites, board of nursing website, textbook, lecture notes..) for this info, but cannot find it! Does anyone have any ideas where this info could be located?

What baffles me about this assignment is that it is asking for "4" elements of a collaborative agreement between an APN and a physician. I don't know about the 4 elements of a collaborative agreement but there are specific elements in the document required by both Medicare and Medicaid for a nurse practitioner to be reimbursed and there are more than 4 of them. In the State of Pennsylvania, the rules on the collaborative agreement document do not just enumerate four elements but actually lists at least six "must includes" as per this link:

Of course I checked the BON website. There is no where "four" elements are delineated. I could find a list of 6 or 7 in my textbook, and as mentioned the BON website listed more than 4.

I think there is actually a "sample" or template of a collaborative agreement posted on the Pennsylvania BON website. What part of PA are you from? Just curious as I am from PA too.

Hi there,

Four elements I have included in mine were:

1) Statement of Services - CNP services (assesses/treat) and prescriptive authority (general statement about having a COA and ability to prescribe - more detail in #4).

2) Incorporation of new technologies or procedures (how you would document them).

3) Quality Assurance Provisions - using referral physicians, how to use my collaborating MD, Chart reviews, how to cover absences

4) Prescriptive Authority - medications, laboratory/diagnostics.

I am from Ohio and although I don't believe there is a dictate as to what absolutely has to be in one...one of our professional organizations has suggested a template that most of us use. If you need more help PM me and I will share more. Hope that helps. - Tracey

Thanks again, everyone. It turned out she said it during a lecture and I missed it. They were broad overall elements:

1. Describe functions/tasks to be delegated (privileges being requested)

2. Time, place, and manner of supervision

3. Location and practice

4. Identify substitute supervisor

I don't think they even cover the things required by PA code (including amount of and other important stuff) but I guess that's what she was asking for.
