   i'm for Bolton NHS. i guess it depends on your trust. you can look at it on your offer letter. in my case the visa and ihs was paid by the trust but there are trust that you'll pay first and make a reimbursement later like onbthe original trust where...

    Ah ok. Well its not necessary as long as its all original. By the way you need also a photocopy of all your supporting documents and make it in A4 size
