Cognitive raps question?


Hey guys only been an MDS for 2 weeks I'm really having trouble writing raps especially the cognitive raps. How would you write a raps for a patient that triggered short term and long term memory problem, moderately impaired in B4(cognitive skills for daily decision making) and C4 coded 2 sometimes understood. CVA patient. any example would be great thank you.

You are making it alot harder than you need to .... the only ones that read them are you and maybe the state assessor if they choose that resident. Make it short and simple- Resident #1(Name) is noted to have short and long term memory problems, and is moderately impaired in his/her decision making and sometimes understood in expressing ones request all as a result of the his/her CVA. Proceed to careplan (where you will make entries on all of these- individualized just to this resident)

I was taught a formula type answer to write RAPS with, it goes like this,
" Triggered by short/long term memory loss, impaired decision making and sometimes understood by others. Mr( Ms) Last Name had a CVA (fill in for diagnoses) and is in facility for short term(long term, rehab, etc.) and add other pertinent info like therapy or meds here. Referrals will be made as needed. I will (will not) care plan this.

No one reads these so killing yourself is a big waste and I have no time to waste. Too much to do!

Thanks guys i'm just worried i'll get in trouble with the state if i'm doing it wrong. It's time consuming writing raps.
