My patient is 74 years old and is cognitive impaired. She has been diagnosed with dementia. Her BP is normal as well as temperature and all other vitals. Objectives are things that you as the nurse can measure and see for yourself such as seeing, hearing, touching, smelling, tasting, measuring, counting, etc. SOme examples are blood pressure, pulse, respirations, bruises, edema, etc.
