I am trying to do a nursing plan and having problems coming up with some objective defining characteristics. My professor keeps telling me what i have is not measurable. My patient is 74 years old and is cognitive impaired. She has been diagnosed with dementia. Her BP is normal as well as temperature and all other vitals. Please helpe me come up with some objectives even if it does not pertain directly to this problem. i need help in realizing what a measurable objective is. Thank you so much.

Objectives are things that you as the nurse can measure and see for yourself such as seeing, hearing, touching, smelling, tasting, measuring, counting, etc. SOme examples are blood pressure, pulse, respirations, bruises, edema, etc.
