Hi and welcome to AN. From one of our favorite posters who loved to help students, Daytonite:

An example would be:

10am
P
- Nausea. "I feel like I'm going to throw up."

I
- Abdomen rounded and soft with bowel sounds all four quadrants. Remains NPO. IV fluids infusing at 100cc/hr. Medicated with Compazine 10mg IM for the nausea at 10am. Encouraged to lie on left side with legs flexed for comfort.

E
- At 10:30am patient was observed resting quietly. Voiced relief of nausea.

Think of it like this....

P: problem-disease process (whats going on with your patient at assessment)

I: intervention-what you did to assist or implement to alleviate the problem at hand

E: evaluation- how your intervention worked
