I'm new to this and wondered if anyone could help as I have a project due and need another nursing diagnosis. My patient has hypertension (HTN), is on a feeding tube (PEG tube) and has dysphagia (so she only drinks a little bit of Frazier Free water daily). I already used a "Risk for" diagnosis with HTN and now need another diagnosis with dehydration somehow related to or maybe seconary to HTN. Any help would be greatly appreciated!
