Does anyone have a list of nursing assesments and the rationale?

Or does anyone know if a book that would explain the nursing assessment/procedure and rationale for doing such assessment?

For example: a patient was diagnosed with r) leg cellulitis:

what nursing assessments would you do?

I would do a wound assessment/pain assessment. Observations to see if he has an elevated temperature?

etc- :) can anyone thing of anything else?

I only know of advanced-level books when people are studying for NP type things but I'm sure there are books that are more basic-to-intermediate type of assessment/interventions/rationales. There was also one everyone was talking about for delegation since that can be a struggle, too. A google search would serve you well but the first one I found was:

See, nursing diagnoses are not as a result of medical ones, and you should be able to do a nursing assessment without knowing the medical diagnosis. Check your physical assessment handbook for how to do them.

Say, what if your patient is admitted with only r/o (rule out) medical diagnoses? Do you stop right there and do nothing until the physician bails you out? No, of course not.

As I lack some knowledge on the basic content of pathopgysiology, does anyone if doing a program like hurst would help at all to revise all those years of information that never really clicked in?

I used to carry those clinical pocket books while in school. Had a pediatric and medical surgical pocket book, made care planning and my life a whole lot easier. Also allowed me to paint a picture of my focused assessments and interventions in clinicals where patients were chosen/vice versa the same day. I'll post here what the book were someday when I'm not lazy.
