Confused on how to do an Elderly Process Recording... help?

i'm doing a process recording on an elderly person, just someone random from the senior center that agreed to sit and reminisce about themselves a bit. the problem i'm having is the introduction. i have the cover sheet, which states the demographics, data, setting, and client-centered purpose, but my problem lies with the introduction. when i turn to the columns, the top of the first one is suppose to have an introduction to the communication that also contains the client-centered purpose of the interview and the time frame. i thought that was what i did on the cover sheet? what exactly am i missing here? this is what my cover sheet looks like:

i interviewed c.k. in the quilting room of the oshkosh senior center during the afternoon. i noticed her folding and putting away some fabric, so i walked over to her, smiled, and introduced myself as i helped with folding the fabric. i then asked her if she would mind spending a few minutes talking with me. i explained i was a nursing student at f.v.t.c., and the purpose of the interview is for me to learn how to implement effective communication skills. we sat at a table, side by side, both of us turned slightly to face each other. there were other people working and talking in the room, yet we were able to have a good conversation at a comfortable volume. the interview went from 12:00pm - 12:20pm.

the client centered purpose of the interview is to promote self-esteem, confidence, and companionship through the use of reminiscence.

is my introduction suppose to be how i introduced myself? i'm so confused, and it's probably something so simple.... any help would be great.

I've had to do process recordings, but not exactly like that. I suspect each school or professor has their own preferred format. You should probably just ask your instructor about it.

If I had to guess I would keep all the data on the cover sheet and add a bit more if you can.... like "C.K. was a petite woman of average weight who used a cane". I'm assuming you didn't see a chart for her, so think about what you assessed based on your observations. Did she seem oriented to reality? confused? was she ambulating well? was she clean, dressed appropriately for the weather, hair combed? Then I would put all of the introductory conversation info in the column - like how you introduced yourself and got the conversation going. Basically get the chitchat out of the way so when you start your verbatim transcription it's the actual therapeutic conversation. But like I said, this is just a guess based on the info you've given. If you want to be 100% sure just ask/email your instructor for clarification.
