Clinical organization sheets

I know I saw these being sent around to organize patient information. Does anyone have one that I can use for the ICU? For ventilator patients? I am only assigned one patient but know my day will very time consuming i want to be organized. Thank you to all that responds. I really appreciate it!!!!!

i have for some time had the link to a student clinical report sheet attached to my signature. click on student clinical report sheet for one patient at the bottom of this post to open it up and print it out. if that doesn't suit your need here are some links to some others i've saved links to over time that might work for you:
