Care Plan 3 part goal statement?

I can't exactly get the 3 part thing figured out. My main diagnosis is pain.

My goal is to have my patient's pain at an acceptable level in one hour.

That states a goal & a measurable time frame, but what am I missing. I've definitely not got 3 parts.

I never have a problem with interventions, rationale, or evaluation. I just don't think I've been asked for the goal to have 3 parts before. What am I forgetting? :confused:

It should look something like this:

(Part 1) Pain, acute (or chronic), (Part 2) related to ............., (Part 3) as evidenced by..............

Your care plan book should tell you about the "related factors" and the defining characteristics.

Your goal statement could be tweaked a little better, too. It needs to be client centered, so instead of "have the pain at an acceptable level", try something like "the client will state relief or decrease in intensity of pain within an hour". That's a measurable goal. The client will either state it or not, and it gives a time frame in which to do it. You'll know at the end of the hour if the goal has been met.

This is correct, of course.

I misunderstood. I thought the OP was asking about diagnoses. I see now that the question was about the goal statement.
