Why spend money on a book when you don't have to? Go to the Merlin nursing diagnosis website. It has every possible nursing diagnosis and the interventions as well. It doesn't cost anything. All you have to do is point and click, and the software will build your care plans for you.

Ackley, Betty & Ladwig, Gail (2004). Nursing Diagnosis Handbook (6th edition).
Sparks, Shelia & Taylor, Cynthia (2001). Nursing Diagnosis Reference Manual (5th edition).
Swearington, Pamela (2004). All-in-One Care Planning Resource (1st edition).

I had a fellow student who so relied on careplan helps, that when the time came to formulate his own diagnoses, he was unable to do so. The reference books had become such a crutch, that he couldn't think of an original diagnosis or interventions to go along with it.
I'm not saying to not get help. I would just suggest using the sources for ideas and try to formulate the needed information on your own.

I have Ackley too. It gives you the NANDA diagnosis and examples of the 5 stages; assesment, nursing diagnosis, goals/outcomes, interventions/implementations, and evaluations.
