Outgoing/Incoming referral

Hello, can somebody please help me understand the referral process at your hospital:

1. Does any of your Inpatient Departments/roles (ER physician, Hospitalist, Dietitian, Ancillary departments etc.) create an outgoing referral/consult for the patient?

2. If answer for above is yes, then how is referrals/consults currently generated? Typically what are the most common services/facilities would those be created for?

3. What are the clinical documents (lab results, progress notes, summary of care etc.) required to be sent out along with the outgoing consult?

4. If patient is referred to outside facility, does hospital have to obtain any pre Auth. number for the patient?

Similarly, for Incoming referral to the hospital

1. Where do you get referrals from today?

2. What are these referrals for (what departments/services)? Like Radiology, PT, OT etc..

3. What do the referral contain (data elements (prior auth number, summary of care documents etc)?
