GOOD nursing care plan books

Can anyone recomend a really good guide to care planning? I do have the sursing DX pocket guide, but I would like to get a handbook on how to write a good, solid care plan. I know this might be my weak spot if I do not brush up. I do a ton of documentation for my job as a visiting nurse, but it's all assess and treat...NOT wirtten out like an actual care plan. I have stong documentation skills, so I know once I get a feel for how to write them I will be OK.

Any advice on good books out there?

Your Fundamentals of Nursing Text will assist you as will the CPNE ™ manual. Every Nursing Program does care planning a little different so you have to use your manual to learn how to do care plans the EC way.
