Well, I just finished reading the survey reports for dialysis units in California.. WOW, even with the new Conditions the mistakes are terrible. I can't understand why? Staff not following policies/procedures of their own unit, infection control deficiencies, mistakes that have placed patients in either situations of potential or actual harm.. I don't get it.. What is happening.

I think you need to be prepared to name names, incidents. To say that the surveys that have been handed out, filled out by pts (not medical people) and their interpretation of events reveal all these mistakes/potential mistakes, etc., is slanderous against an entire industry without facts to back it up.

That is not what I said. I think you misinterpreted. I am speaking of the 'surveys' that are conducted by the state on behalf of CMS. These are called 'surveys'. The facilities are inspected by surveyors in each state. Then depending upon their patient/staff interviews, medical record reviews and observations, they write what is called "The Statement of Deficiencies" Form F 2567. Then this is given to the facility and the facility has to write a POC (plan of correction). I am not speaking of patient surveys and sorry that you misinterpreted that which I wrote. The surveys are posted on a website, however, the rules of this forum are that one can not post any websites, otherwise, you would be able to read these surveys. These surveys, in all states, are public records and can be obtained by anyone. Some states as PA and CO post surveys to their websites (the department of health websites). Most states do not post these surveys. I hope that makes what I stated a bit more clear.

As an added note, I dont think that i stated anything was filled out by patients and mentioned Conditions. I also did not state that surveys were handed out, nor did I say anything about a patient's interpretation. I think, in all due respect, that you read into what I wrote. I just wanted to clarify an again sorry that you misunderstood. There are some very good, excellent facilities but there are also some serious deficiencies that are made in some units. When Conditions are NOT met versus a Standard that is not met... then that is not okay.

No problem. I guess my passion for quality safe care is more than I can state. I have been deeply involved in advocacy and I truly know that there are facilities that provide the best care for their patients. Then there are units that have staff that care and show such, and then there are staff in the same unit that don't care. It is these few that can ruin a unit and then new staff follow suit.

My goals are to have true patient-centered care, not just on paper, and that all staff understand what patients experience both emotionally and physically... Indeed some jus turn their lives over the staff and that is ok, but there is a large population that given the chance want to be involved, that is, if they are approached by staff in an inviting way,
