Pain management in Post-CABG patients

I am interested in hearing about what other hospitals are doing to manage pain in post-CABG patients. The hospital that I work for is working on a new pain management protocol and I am interested in pain management specific to my unit (CVICU). Thank you for your input!

CVICU...post CABG... MS IV q1-2 prn while on vent....Diprivan gtt per protocol while on vent...titrate to sedation....post extubation....percocet 1-2 tabs q4-6 prn....MS 4-6 mg IM q3h PRN Toradol 30mg IM x1 and 15mg IM q6h x4 PRN if platelets >100,000, normal creatinine, BUN. :kiss

We use Diprovan while intubated. When CABG pts are extubated we use Morphine q 2 hrs while in the unit. We usually give Toradol when we reverse, if the patient doesn't have a hx of ulcers.
