Hello, I am new to allnurses.com and am very interested in hearing feedback regarding Disease management case managers. I have been offered this position and am unsure if this is right for me. Please tell me a little about what I can expect. Please share good and bad. Thanks. Hope to hear from you soon. I have to make a decision soon.

I worked in a Disease Management position for a year. I guess I am a case manager at heart because I found it frustrating just listening to all my patients' problems without being able to help them. I am now working as a case manager in a program that has a strong Disease Management component. Disease Management is considered Population Management - one of the new Healthcare buzzwords. Many companies and governments agencies are finding that managing the "heavies" like Asthma, Diabetes, and CHF, will prevent inpatient stays and other high-cost events. The numbers support this, so I believe everyone will be using Disease Management. I work for my State Medicaid Office and it has been a very successful part of our program.
