I've recently gotten a job in field case management helping chronically ill patients who are with a certain insurance provider. My first position in this field; I don't even have home health case manager experience. Any words of advice, or resources to look up? Would it help to get some kind of nursing care plan book to help with making action plans?

Why do you need Home Health Case Manager experience? Home Health Case Managers are actually Home Health Nurses rather than Case Managers that are represented on this forum. So, are you a Home Health Nurse? Or are you a field Case Manager similar to Workman's comp Case Managers or Medicaid Case Managers?

If you are a Case Manager working for a company similar to Workman's Comp or Medicaid Case Managers, then this is the place to be because this forum is filled with many nurses who can answer your questions and concerns.

Otherwise, if you are actually a Home Health Nurse (Many agencies give Home Health Nurses the title of Case Manager but they are not case managers in the same sense as those on this forum), you may want to try the Home Health Nursing forum: Home Health Nursing

Thanks for asking! I'm not a home health nurse, but a field case manager working for an agency that is contracted for Humana. It's not workman's comp, and not working for Medicaid. (Most of it will be visiting in the field doing assesments, connecting people to resources, etc., but no hands-on nursing). Doesn't seem like there will be a lot of orientation.

Humana has many Social Workers and you should have one on your team... they can help you with resources. Have you checked with them? Ask them where they get their resources. Also, Humana has many resources for you online. Good luck..

Thanks, Rntr and MBARNBSN! Yes, I saw the online resources at Humana. Haven't talked to any social workers yet (probably none at our agency) but I saw Humana does have a central resource specialist that can be communicated with.
