How are your hospitals handing issues with meds given "wrong route" when an NG/OG/PEG tube is present and some meds are ordered PO, and nurses are crushing them and giving via tube? For example, if a patient gets intubated, who is responsible for changing all their PO meds to "via tube" route? Does the provider get a pop-up from the ordering screen? Does your pharmacy make the changes? Looking for ideas! Any are appreciated! Thank you!
