Will I need a refresher course?

I am a new-ish nurse, I have 1.5 years of experience which includes 6 months in an extremely busy rehab facility where I utilized a lot of skills, and now 1 year in a peds clinic where I use a different set of nursing skills. I really like where I work, the doctors are wonderful and the nurses/MAs are great as well. It is an incredibly busy environment and I do very heavy phone triage, tend to emergencies about once/twice per week, give meds/treatments and deal with all labs/pt. teaching. But it is not the same skill set as an acute care setting, obviously.

I dont want to leave after only 1 year, it is a great place to work/is a healthy and caring environment, I would feel guilty leaving so soon, but I feel I should be working in an acute care setting to really learn/grown as an RN. I particularly am interested in a children's hospital/pediatric unit with my peds experience. I want to give it at least a year or two from now before moving on though.

Two years from now if I am still working at this peds clinic, will I need a refresher course by then to get into the hospital? What should I do now to help my chances? I looked into going per diem at a hospital now while working at the clinic, but they would need to train someone full time before going per diem, which is not surprising. I am full time at this clinic so that wouldn't work.
