I sign my name in one fell swoop. There's an old state form I used nearly 30 years ago when I worked at the state hospital that is still used where I am presently employed. It's that's called a Restriction of Rights form.
Like most forms generated by the government, it's illogically redundant, having probably a half a dozen places for the author of the form to sign, so my signature often looks like this:
If the patient wants a copy of the form, I change my signature to this, so they don't know both my first and last name:
Like computer passwords, I have different signatures for different types of forms.

Do you sign documents not job related with RN like personal documents? I know this maybe a stupid question but I was just wondering!
