Transferring HESI scores

Does anyone know how to send their HESI scores and how much it costs? I can't find any information on it. Sending my HESI scores to multiple schools so would like to know how much money I should expect to spend.

For every transcript/school you have to pay $15. Once you check out (order is complete), you get your order number. With that order number you fill out this form:

http://coursewareobjects.elsevier.com/objects/Fulfillment_Exports/Customer_Files/HESI/HESI_Transcript_Request_Form_V2.pdf

After filling it out, you send it to [email protected] who after receiving your email (so try doing it right away) with the attached file, proceed to send your transcript to said school.
