Totally dependent, total care pt. care plan outcomes??

We were told not to do Goals but Outcomes with our total care pts. because goals are not attainable...their conditions do not improve. What are some outcomes I can use, for example, with Self Care Deficit as the Nursing Diagnosis. Something like "Pt. will be free skin breakdown, falls, infection, etc.....................?????? Is that acceptable?

Yes, outcomes always should be show free of skin breakdown, falls, infection, but in a specific time i.e by end of shift, till discharge, in one week, or in one month depend the pt's condition.
