Total Hip Replacement Care Plan

I am doing a random care plan that is to have 2 nursing dx with as many outcomes and interventions as need for a Total Hip Replacement. I choose Self Care Deficit (toileting, bathing and hygiene) r/t mobility restrictions and Risk for altered respiratory function r/t immobility. I need help with expected outcomes. How do you know when you have enough. I can't have less then 5.

How many outcomes have you come up with so far? It's much easier to help when we know how much time and effort you have already put into this.

If you haven't come up with any, I suggest you consult your NANDA Nsg Dx book. I'm sure you'll find your information there.
