According to the lady who administered our test, a BIG thing that causes people to fail is safety risks. For example, raising a bed to give a bed bath, then walking away, leaving the bed raised. Or rolling someone without having the siderail raised, etc. In some states, safety violations are immediate fails, even if you've done the actual steps of the skills perfectly.
