Survey help!!

Does anyone have any suggestions for how to best conduct a survey to nurses or physicians? I've used this forum with no replies whatsoever. I desparately need 150 responses on a survey regarding Infectious Diseases that takes less than 3 minutes.

I have been conducting this survey via the phone and have only gotten a small amount of response.

I'll try and post it again. Only problem is that it's 5 questions and the best way to conduct it is to talk to people live.
