Suggestons for ER miscarriage

I work in a very small ER we have no OB service but sometimes get patients that are having miscarriage. I would like to put together a small kit to keep in our GYN room that would have things needed for lady partsl exam and would like any suggestions that the experienced nurses of this forum could suggest. Obviously ring forcepts, formulin, etc. Also could you suggest what you might think should be kept in the GYN room for routine pelvic exams for pregnant women. I know this is alot to ask but I am trying to make things more efficient in this ER. Thank you in advance.;)

You might also want to have a couple different sizes of speculum (you may already have them if it's the GYN room), some KY (ditto), a box of Kleenex (she may be upset/crying). That's all I can think of for the moment but I'm sure some other posters have great ideas too. Hope this helps, and kudos to you for wanting to improve things!

You might wanna have on hand some cytotec, for a missed AB with bleeding. You may also get the occasional 'much farther along than she thought' patient and you might not have time to transfer her before the big show. I'd also recommend an emergency ob kit. Ours come in little boxes, maybe 10x4 inches. They have a couple chux, 2 blue OR sterile towels, sterile gloves, bulb syringe, baby blanket and hat, 2 cord clamps (if you really don't have those in your hospital, use a couple sterilized curved kellys instead) sterilized scissors. That should handle the delivery until squad comes to transfer her. We keep a couple stocked in our ER too, just in case they get an active one down there.

Does your facility's blood bank carry RhoGAM? Any missed AB whose blood has an RH - factor, must recieve RhoGAM to counter the RH antibodies that have the potential to abort the next pregnancy.

As someone who has personally suffered miscarriages, 4, not only would I suggest you do these things but it would not hurt to see if you can get some type of counselor to be on call, or a nurse who has been certified in dealing with this type of loss. The grief hits immediately, although the total loss, at least in my case, did not fully impact me for a few days. I was in TOTAL shock, wouldn't talk, really needed someone to explain to me that this was just something that happens, it isn't necessarily the mothers fault, etc.etc. I think this is just as important as having the right "tools" around. I am just speaking from personal experiences and what I remember really needing was someone to talk to me and explain. Anyways, my 2 cents.
