Student evaluation of clinical instructor

I am part way through my first experience in clinical teaching. I would like the students to evaluate my performance and effectiveness at the end of the practicum. Does anyone out there have an evaluation tool that I could give to the students for this purpose? Even a list of questions or categories would be appreciated, and I could make my own tool.

I worked for a national company employing nurse educators. Their tool was divided into sections such as

knowledge of subject/clinical expertise

presentation skills/organization of material (this could be about your pre and post conferences)

availability to students (on the unit) - aim to see each student every hour at least

ability to see every student as having potential/equal treatment - I forget the exact wording

Comments/include specific instances of helpful/unhelpful practice

These forms should be handed in to clerical assistants in your school - you should not see the originals or the students will be afraid to be critical fearing repercussions - no matter that you would not do this. The asst. should type up the results and comments for you to keep.

The org. I worked for threw out the least and most glowing reports - anything from 1-3 dep. on # in group. Grade on a scale 1-4 - forces choices - is good; but 1-5 is OK.

I am surprised that your school does not have an evaluation tool already in use. Do check with your clinical coordinator before you get egg on your face. This is usually required for BoN approval of the school program.
